Position Summary
The Case Manager coordinates discharge planning, aftercare referrals, and assists patients in accessing necessary benefits to support their recovery and well-being. This role collaborates closely with clinical and nursing teams to ensure comprehensive, whole-person care and supports patients’ transitions to community resources and ongoing treatment.
Key Responsibilities
- Coordinate discharge planning and facilitate aftercare referrals to ensure continuity of care.
- Assist patients in accessing benefits including Oregon Health Plan (OHP), Supplemental Nutrition Assistance Program (SNAP), and Social Security Income/Disability Insurance (SSI/SSDI).
- Maintain accurate and timely documentation of case management activities in compliance with OAR 415-050.
- Collaborate with clinical, nursing, and support teams to ensure integrated, holistic care for patients.
- Support families, caregivers, and outside providers during patient transition planning and ongoing care coordination.
Minimum Qualifications
- Bachelor’s degree in social work, behavioral health, or a related field; OR
- Certified Alcohol and Drug Counselor (CADC) Level I or higher; OR
- Qualified Mental Health Associate (QMHA) certification by MHACBO.
- One (1) year or more of experience in case management or social services preferred.
